My guess is that anti-psychotics aren't likely to have much of an effect on tulpae unless you believe they will, hence why it only affects tulpae of some hosts and not all. That said, there aren't very many accounts of this at all, so we don't really know. Deluded myself into believing my imaginary friends were real, then deluded myself into thinking they weren’t.
