Okay, so the weird one first: What to do if your mind is actively working against you?

If this happens, it means you're getting intrusive thoughts. The best thing to do is to clear your mind, try to stop whatever is changing, and bring your focus back to whatever it was you were doing. This guide list has a section for concentration. Maybe you'll find some exercises there that could help you out.

 

The second question is more normal, but also more serious. Ive read in multiple guides that total concentration is necessary to create a tulpa. Is that true?

If you're only able to do fifteen to twenty minutes of active forcing at a time, that's alright. You don't need to be doing this for hours on end. You can space sessions out during the day, so whenever you have a little bit of quiet time, try to use it to work on your tulpa. And when you're not taking the time to focus only on your tulpa, you can narrate.
