If you haven't already, check out the dot exercise. There's also a guide by TOG, which basically suggests taking breaks.

 

I suggest practicing a basic meditation technique like controlled breathing and progressively relaxing your body. All of this takes practice.

 

I started out with incredibly short sessions as well; it just got better with time. Typically if I got frustrated with being unable to concentrate, that meant the session was pretty much over until I calmed down. Frustration kills any sort of concentration you hope to accomplish.
